VISTA — The Self-monitoring, Treatment, Education, Physical activity, Sensible eating (STEPS) program is being offered to Vista Community Clinic patients with chronic disease including diabetes, hypertension and hyperlipidemia.The purpose of STEPS is to provide patients with quality, affordable chronic disease care, including education for prevention of complications caused by chronic disease.
STEPS is available to any patient diagnosed with a chronic condition listed above who is insured, uninsured or underinsured. If a patient does not have insurance, a sliding scale fee structure is available to them based on their income and family size.
STEPS services include consultation with a physician, consultation with nutritionist, lab work, Health Education classes and an annual retinal exam.
Patients with a chronic condition can be referred into the STEPS program by a Vista Community Clinic Provider or staff person. If you would like more information on STEPS, contact Raquel Aguirre at (760) 631-5000 ext.1102.
